User manual for the app 

In the Customer Feedback application,fields which are marked with a star "*" are mandatory fields and the customer cannot skip these fields. Fields having Calender, or a Dropdown list, there only one entry can be selected by the user. As the customer submits the data by clicking on the 'Submit' button entire data will be recorded in the Database, and the customer will get a confirmation email on the email id provided by him/her.

    1. Name of product 
    2. Response of product 
    3. Will you like our product 
    4. Will you like to purchase our product in future 
    5. Would you like to get notification regarding discount/offer in future 
    6. Contact Details of the customers 
    7.Time limit should be within working hour(10 am to 6 pm) 8. Upload image (for system record) Image              can be uploaded by various way Source, alt Text, Link, Title Or it can be directly uploaded.

c.       Rating for the quality of the food provided
5)      If chosen for room it should ask

After completion and submitting the form the responder should get a thanking from the company which similar to an auto response

Initially there should be no options available than feedback for and then based on choosing the option it should display the corresponding feedback question.
