This is form meant for our hotel and restaurant services. The form should get the following datas

1)      Name

2)      Contact Number

3)      Feedback for

a.       Restaurants

b.      Rooms

4)      If chosen forrestaurants it should ask

a.       Table number

b.      Rating for menu available

c.       Rating for the quality of the food provided

5)      If chosen for room it should ask

a.       Room number

b.      Type of room

c.       Rating for

                                                               i.      Room cleanliness

                                                             ii.      Room Service

After completion and submitting the form the responder should get a thanking from the company which similar to an auto response

Initially there should be no options available than feedback for and then based on choosing the option it should display the corresponding feedback question.
